Zepbound is for adults with obesity (BMI 30 or greater) or overweight (BMI 27 or greater) who also have at least one weight-related condition, such as high blood pressure, type 2 diabetes, abnormal cholesterol, or obstructive sleep apnea. It is used with a reduced-calorie diet and increased physical activity.

People who should not use Zepbound include those with a history of medullary thyroid carcinoma, MEN2, or a prior serious allergic reaction to tirzepatide. It is not recommended during pregnancy. Dosage and Usage

Zepbound is injected subcutaneously once weekly, on the same day each week, with or without food. Rotate injection sites in the abdomen, thigh, or upper arm.

Common titration plan (follow your prescriber’s directions):

  • Start: 2.5 mg once weekly for 4 weeks
  • Increase to 5 mg once weekly for at least 4 weeks
  • Then increase by 2.5 mg steps at intervals of at least 4 weeks as needed: 7.5 mg, 10 mg, 12.5 mg, then 15 mg once weekly

Maintenance dosing is individualized. Some people do well at 5 or 7.5 mg. Others may need higher doses for additional effect, if tolerated. If gastrointestinal effects occur during titration, your prescriber may hold at the current dose longer before increasing again.

Strengths and Forms

Zepbound pens are single-dose, prefilled autoinjectors. Common strengths include:

  • 2.5 mg/0.5 mL
  • 5 mg/0.5 mL
  • 7.5 mg/0.5 mL
  • 10 mg/0.5 mL
  • 12.5 mg/0.5 mL
  • 15 mg/0.5 mL

Missed Dose and Timing

If you miss a dose, take it as soon as possible within 4 days (96 hours) after the missed dose. If more than 4 days have passed, skip the missed dose and take your next dose on the regularly scheduled day. Do not take two doses within 3 days of each other. Storage and Travel Basics

Store unused Zepbound pens in the refrigerator at 36°F to 46°F (2°C to 8°C). Do not freeze. Keep pens in the original carton to protect from light. If needed, you may store a pen at room temperature up to 86°F (30°C) for up to 21 days. Discard pens that have been frozen, overheated, or left out past 21 days at room temperature.

Pen Handling and Sharps Disposal

Inspect the pen before use. The solution should be clear and colorless to slightly yellow, without particles. Do not use if damaged or expired.

  • Wash and dry hands.
  • Choose an injection site and clean it with an alcohol swab.
  • Attach a new needle if required, and follow the pen’s instructions for priming if applicable.
  • Place the pen on the skin at 90 degrees, press and hold as directed until the injection is complete.
  • Keep the pen pressed for the required count so the full dose is delivered.
  • Remove and dispose of the needle in an FDA-cleared sharps container.

Low blood sugar can occur when used with insulin or a sulfonylurea. Seek medical help for severe abdominal pain, persistent vomiting, signs of gallstones, or symptoms of allergic reaction.

Drug Interactions and Cautions

Zepbound delays gastric emptying. This may affect how your body absorbs some oral medicines. Estrogen-containing oral contraceptives may be less effective during initiation and after dose increases. A non-oral or backup method is typically advised for 4 weeks after starting and for 4 weeks after each increase.

Using Zepbound with insulin or a sulfonylurea can raise the risk of hypoglycemia. Your clinician may adjust other medicines to reduce low blood sugar risk. Do not combine Zepbound with other GLP-1 receptor agonists or other tirzepatide products. For example, do not use it together with Mounjaro® Kwikpen. Tell your clinician about all medicines, vitamins, and supplements you take.

What to Expect Over Time

Many people notice reduced appetite within the first weeks. Gastrointestinal symptoms are common during early dose steps and often lessen over time. Weight change is gradual and varies by dose and adherence to diet and activity. Your clinician will review progress and adjust the plan as needed.

    2 pens failed
