Your clinician may increase the dose in 50 mg steps, no more often than every 3 days, to improve pain control and tolerability. Many patients stabilize between 100 mg and 250 mg every 12 hours. The maximum recommended total daily dose is 500 mg.

Swallow tablets whole with water. Do not cut, crush, split, or chew. Doing so can release a potentially fatal dose. Take at the same times each day, with or without food, consistently.

If switching from another opioid, your prescriber will determine the starting dose. Do not convert on your own. Do not combine with other tapentadol products unless your clinician directs it and monitors you.

Avoid use with MAO inhibitors or within 14 days of stopping an MAOI. Use great caution with benzodiazepines, sleep medicines, muscle relaxants, or alcohol. These raise the risk of dangerous sedation and respiratory depression. Serotonergic medicines, such as SSRIs, SNRIs, TCAs, triptans, and some migraine or Parkinson’s drugs, may increase the risk of serotonin syndrome. Mixed agonist/antagonist opioids such as buprenorphine or nalbuphine can reduce analgesia or precipitate withdrawal. Anticholinergic drugs can worsen constipation or urinary retention. Tell your clinician about all medicines and supplements you take.
