Dosage and Usage

Standard initiation follows the FDA- and Health Canada–approved schedule to help limit stomach side effects:

  • Start 0.25 mg once weekly for 4 weeks. This is a starting dose, not for glycemic control.
  • Increase to 0.5 mg once weekly for at least 4 weeks.
  • If additional control is needed, increase to 1 mg once weekly.
  • If needed, a further increase to 2 mg once weekly may be used (maximum labeled dose).

Inject on the same day each week, with or without food, at any time of day. Common sites include abdomen, thigh, or upper arm. Rotate injection sites. If used with insulin or a sulfonylurea, hypoglycemia risk increases.

Missed Dose and Timing

If a dose is missed and the next scheduled dose is more than 5 days away, take the missed dose as soon as possible. If less than 5 days remain until the next dose, skip the missed dose and resume your regular schedule. Do not take two doses on the same day.

You may change your weekly dose day if needed. Ensure at least 48 hours have passed since the last dose before switching to a new weekly schedule.

Storage and Travel Basics

Before first use, keep pens refrigerated. Do not freeze. Protect from light by keeping the cap on. After first use, store at room temperature or in a refrigerator. Keep away from heat. Discard the pen after 56 days, even if medicine remains.

Pen Handling and Sharps Disposal

Wash hands and gather supplies. Attach a new sterile needle each time. Perform a flow check the first time you use a new pen, per the instructions. Dial to your prescribed dose. Insert the needle into the skin. Press and hold the button until the dose counter returns to 0, then keep the needle in place for a few seconds to ensure the full dose.

Remove the needle right after injection. Recap the pen and store without a needle attached. Dispose of needles in a puncture-resistant sharps container. Follow local rules for disposal. Do not throw loose needles in household trash.

Drug Interactions and Cautions

Using Ozempic with insulin or a sulfonylurea can increase hypoglycemia risk. Dose adjustments of the other agent may be considered by your prescriber. Avoid combining with other GLP-1 receptor agonists. Because semaglutide slows stomach emptying, be cautious with drugs needing fast absorption or having a narrow therapeutic index. Warfarin users may need INR monitoring when therapy changes. Discuss pregnancy, breastfeeding, pancreatitis, gallbladder disease, kidney disease, or retinopathy with your clinician before starting.

What to Expect Over Time

Gastrointestinal effects are most common during the first few weeks and often improve as dosing stabilizes. Many patients notice improved fasting and post-meal glucose within weeks, with A1C changes taking several months. Weight changes vary. Your prescriber will review glucose logs and overall goals before adjusting the dose.
