What Januvia® Is and How It Works

Januvia (sitagliptin) is a DPP-4 inhibitor for adults with type 2 diabetes. It helps lower blood sugar by increasing incretin activity, which boosts insulin release and reduces glucagon after meals. Common strengths include 25 mg, 50 mg, and Januvia 100 mg tablets. It is taken by mouth once daily, with or without food, and is weight neutral.

CanadianInsulin is a prescription referral service. Prescriptions are verified with your clinic, and orders are filled by licensed Canadian pharmacies.

Januvia aids fasting and post‑meal control and is often used with metformin or other agents. Typical A1C reduction is about 0.5% to 0.8%. Learn more about its class in Januvia Drug Class How Dpp 4 Inhibitors Support Blood Sugar Control. Januvia is not for type 1 diabetes or diabetic ketoacidosis.

Dosage and Usage

The information below is educational and does not replace prescriber instructions. For practical tips on dose adjustments, see Fine Tuning Januvia Dosage Tips For Personalizing Your Diabetes Care Plan.

  • Usual starting and maintenance dose: 100 mg once daily.
  • Kidney function dosing: 50 mg once daily for moderate renal impairment; 25 mg once daily for severe impairment or end‑stage renal disease (including dialysis).
  • Administration: swallow tablets whole with water; may be taken with or without food.
  • Co‑therapy: when used with insulin or a sulfonylurea, a lower dose of the background agent may reduce hypoglycemia risk.
  • Missed dose: take the next dose at the regular time; do not double doses.
  • Monitoring: glucose trends and kidney function are typically assessed at routine visits.

Storage and travel tips:

  • Store your tablets at 20–25°C (68–77°F); short excursions 15–30°C (59–86°F) are acceptable.
  • Keep the bottle closed to protect from moisture. Leave desiccant in place if present.
  • For travel, carry your medication in hand luggage with a copy of your prescription.
  • Avoid heat, freezing, and direct sunlight. Do not store in a bathroom.
  • If a tablet is damaged or discolored, do not use it; check your supply before trips.

Benefits and Savings

Januvia helps reduce A1C and post‑meal glucose while remaining weight neutral. It is taken once daily and can be used with metformin, SGLT2 inhibitors, insulin, or sulfonylureas. The risk of hypoglycemia is low when used alone or with agents that do not cause hypoglycemia.

Many customers save 60–80% vs typical U.S. prices. See our promotions page for current offers, including any januvia coupon if available.

Side Effects and Safety

  • Common effects: upper respiratory symptoms (runny or stuffy nose, sore throat), headache, mild stomach discomfort, diarrhea, or nausea.
  • Less common: rash or itching, dizziness, or mild swelling.
  • Low blood sugar is more likely when combined with insulin or a sulfonylurea.
  • Severe joint pain has been reported and may start suddenly.
  • Skin reactions such as bullous pemphigoid can occur; blisters require prompt medical attention.

Serious but rare risks include pancreatitis, hypersensitivity reactions (anaphylaxis, angioedema), and kidney function changes. Stop the medication and seek care for severe abdominal pain, allergic symptoms, or blisters. Discuss heart failure symptoms if present, especially when used with other diabetes medicines. Review full safety information with a healthcare professional.

Onset Time

Many patients notice improved fasting and post‑meal readings within 1 to 2 weeks as daily dosing continues. A1C reductions accrue over 8 to 12 weeks, with full effect often seen by 24 weeks. Weight typically remains stable. Consistent dosing and background therapy optimization help sustain results.

Compare With Alternatives

Januvia is an oral DPP‑4 inhibitor that is weight neutral and given once daily. Alternatives include GLP‑1 receptor agonists and a GIP/GLP‑1 agent:

Rybelsus® (oral semaglutide): a daily oral GLP‑1 option. It often yields larger A1C and weight changes but may cause gastrointestinal effects. It has a contraindication for personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia type 2 (MEN 2).

Trulicity® (dulaglutide): a once‑weekly injection. It can provide robust glycemic and weight effects with similar GLP‑1 class precautions, including the boxed warning regarding MTC and MEN 2.

Mounjaro® (tirzepatide): a once‑weekly GIP/GLP‑1 agent with significant A1C and weight reductions in trials, also injectable with GI tolerability considerations and the GLP‑1 class thyroid tumor warning.

When choosing among these, considerations include oral versus injectable dosing, desired weight effects, tolerability, and existing conditions. Discuss options with a clinician to align therapy with goals and preferences.

Combination Therapy

  • Metformin plus sitagliptin: common first add‑on for post‑meal control and A1C reductions.
  • SGLT2 inhibitor plus sitagliptin: complementary mechanisms for glycemic control.
  • Basal insulin plus sitagliptin: may improve post‑meal control; reduce insulin dose to lower hypoglycemia risk if needed.
  • Sulfonylurea plus sitagliptin: consider lowering the sulfonylurea dose to reduce hypoglycemia.
  • Do not combine with another DPP‑4 inhibitor.

Patient Suitability and Cost‑Saving Tips

Potential candidates include adults with type 2 diabetes seeking oral therapy, weight‑neutral effects, and low hypoglycemia risk when used without insulin or a sulfonylurea. Not appropriate for type 1 diabetes or diabetic ketoacidosis. Caution is advised in patients with a history of pancreatitis or serious skin reactions.

Dosing depends on kidney function. Those with moderate to severe renal impairment often need a reduced dose. Review all medications and allergies to check for hypersensitivity risks. Pregnant or breastfeeding individuals should consult their clinicians to weigh benefits and risks.

Ways to manage costs:

  • Consider a 90‑day supply to reduce per‑tablet costs and shipping events.
  • Align refills with other medications to consolidate shipments.
  • Check available strengths to match the prescribed dose and avoid waste.
  • Set gentle reorder reminders so you do not run out during travel.
  • Explore seasonal promotions when planning refills.

    New to Januvia

I started taking Januvia after trying other medications which upset my stomach or gave me low blood sugar. Within 2 weeks I got past the very slight stomach upset and have had no problems taking this medication.
