You are going to have a clinical encounter using videoconferencing technology. You will be able to see and hear the provider and they will be able to see and hear you. The information you share with the provider may be used for diagnosis, treatment, therapy, follow-up and/or education.

Therefore, I understand that while telemedicine may allow a provider to diagnose and provide treatment plans for many medical conditions, some conditions may require further diagnostics, or an in-person visit with a provider.
3. I understand that I may expect the anticipated benefits from the use of telemedicine in my care, but that no results can be guaranteed or assured.
4. I understand that the laws that protect privacy and confidentiality of medical information also apply to telemedicine.
